They did not actually force people to walk the plank.
Although walking the plank is common in modern pirate lore, most pirates simply killed people. When they tortured their prisoners, it was usually by keelhauling (dragging a tied sailor in the water from the back of the ship), marooning them on a deserted island or remote sandbar, or lashings with a leather whip.
The Jolly Roger flag was not used by every pirate ship.
Pirates from The Bahamas had their flags custom-made by a sailmaker's widow who accepted brandy as payment, while Blackbeard had his own black flag with a skeleton with horns. The skeleton held an hourglass in one hand and a spear pointing to a heart dripping with three drops of blood in the other.
Books have a more forgiving return policy than a January Kohls.
Books are an interesting market in part because they offer a 100% return policy. So, if a publisher sells 50 copies of a fresh new book to, say, Target or a local independent bookshop, that company has the option of returning all 50 copies to the publishing house if they don't sell. Isn't that crazy? 50 copies aren't much, but when you get into the hundreds, you can see how this can be a difficult market to predict.
Authors nearly never create the cover and frequently have no say in the matter.
The same goes for the title. They will almost certainly be given some level of approval as a courtesy, and if they dislike the design or the title, that will be taken into account as well. But in general? It's not their place.
Perhaps it appears ridiculous that an author, who has conceived up an idea, written thousands of words, and gone through several rounds of edits to bring their idea to the world, will have little voice when it comes to packaging up the final book. But, to that, I say go to Amazon and look at some of the self-published book covers and titles for sale. You'll undoubtedly find a few good ones, but the vast majority look like they were created in Microsoft Word
